Discharge Planner
Job Summary:
Assists and facilitates individualized, goal directed discharge processes in accordance with departmental and hospital policies and procedures.
Minimum Education:
-Graduate of an accredited school of nursing, required.
Minimum Experience:
-Demonstrated proficiency in acute care nursing, knowledge and skills.
-Demonstrated proficiency in individualized discharge processes.
Required Licenses/Certifications:
-Current RN Idaho license.
-Current C PR certification.
-LSW or BSW (required for social work employees)
Required Skills, Knowledge, and Abilities:
-Knowledge of local and regional programs to assist in transfer of care to patient’s home or other patient care programs.
